
BOROUGH PARK — Maimonides HEALTH HAS RECEIVED $250,000 from the Fan Fox & Leslie R. Samuels Foundation, Inc., to support advanced illness management (AIM) and age-friendly care. This new grant will be utilized to enhance Maimonides’ existing program for emergency and admitted patients, assist the skilled nursing facilities in becoming age-friendly, and create a new program to support caregivers of our older adult patients. The funding, to be disbursed over two years, brings the total to more than $1 million that has been granted to Maimonides for AIM and age-friendly projects since 2022. This achievement recognizes clinical care settings that have implemented and continuously practice evidence-based interventions for all older adults.
Maimonides’ AIM and age-friendly programs were initiated in 2021, significantly improving the hospital’s expertise and capacity to support the unique healthcare needs and challenges of older patients and those with advanced-stage illness. The Fan Fox & Leslie R. Samuels Foundation supported the initiation of this program in January 2022. In June 2023, the Institute for Healthcare Improvement (IHI) recognized Maimonides as an “age-friendly health system committed to care excellence.”
